Patients may self-refer or physicians can make a referral
SUSAT enables family physicians and primary care clinics to quickly and easily refer people for community-based addictions services such as: counselling group therapy, opioid agonist treatment (OAT), as well as residential treatment, detox and other specialized services.
The team provides:
- Information
- Assessment, matching and referral
- Brief interventions
- Transitional support among substance use programs
- Concurrent disorders engagement and referral
- Rapid re-engagement
- Support and referral for those who are affected by the substance use of others
A referral letter should include: patient contact info, current list of medications, referring clinician's name, and which of the following are of interest to the patient:
- Withdrawal Management (Detox)
- Outpatient Substance Use Counselling
- Residential Substance Use Treatment
- Opioid Agonist Treatment (Methadone, Suboxone)
- Other
SUSAT North and East locations is available 8:30am-6pm Monday to Friday, and the Surrey location is available 8:30am-6pm 7 days a week.
